Step 3: For your intake, please bring the above completed forms and a copy of your insurance card (or take a picture of it and email it to nsc.jeni@gmail.com). Please include FRONT AND BACK.
Step 4: Please be on time for your appointment as we have a lot to get through. When you enter the building, you do not need to check in at the front desk, feel free to have a seat in the lobby and I will be with you shortly.
During your intake we will discuss confidentiality, payment agreements, and answer any questions you might have about your treatment paperwork. The majority of the time will be spent discussing symptoms, history, treatment goals and we will begin the process of treatment planning.
